For your safety and the safety of others, if you are exhibiting symptoms of COVID-19, have been exposed to a confirmed case, or are a resident/worker in a setting that has a COVID-19 outbreak please DO NOT book an appointment, or please contact us to cancel an existing appointment.

Some things to expect:

• Please print and fill out this screening form and bring with you

• You may be required to wait in your vehicle until you are able to be seen

• You will be screened at each visit. 

• All surfaces and equipment are thoroughly disinfected after each use; masks are worn while treatment is being provided. 

• All individuals are expected to wash hands and sanitize before treatment

• All individuals are required to bring and wear a clean disposable or reusable mask for the duration of the time in the clinic and throughout the treatment
